EVA U.S. service-dog flights: prepare the correct forms

For an otherwise qualifying trained service dog, including a psychiatric service dog, EVA's published procedure asks you to send the required forms to Reservations at least 48 hours before departure. EVA says it notifies passengers when approval is given; sending paperwork alone is not that notification.

Important unknowns

  • The source does not establish the outcome of an individual request. Confirm current instructions with the airline; the effective date of these provisions is not established.

What to do next

  1. Identify the account

For flights to or from the United States, prepare the DOT Air Transportation Form. If the flight lasts at least eight hours, EVA also requires the Relief Attestation Form.

  2. Use the official recovery route

Treat 48 hours as EVA's stated submission procedure, not a universal legal rule allowing refusal of every later request. Ask Reservations how your booking will be handled. Check destination and transit entry documents separately: airline assistance approval does not establish that the dog meets U.S. entry requirements.
